Family, Friend, and Self-Referrals

Park Center is able to take referrals from any individual or support person. We welcome you to reach out to us to make a referral or ask any questions.

3 easy options to send in referrals:

1.  If you would like to make a referral to our drug and alcohol treatment program, dial 615-242-3576 and press 2.

If you would like to make a referral to any of the following programs: PSR Day Program, Supported Employment, SOAR, or Supported Housing, dial 615-242-3576 and press 3.

2. You may walk into our location at 948 Woodland St. to meet with our Admissions Coordinator. We accept walk-ins Monday-Friday 9:30am-4:30pm.

3. You may fill out our short online form below to receive a call or email with additional information.

Our Admissions Coordinator will reach out to you directly within 24 business hours. You will then be walked through the next steps of our admissions process.

Permanent Supportive Housing (PSH) referrals are currently on hold.
