Empowered wellness coaching through peer support.

This program is staffed by coaches who have personal experience navigating behavioral health conditions.

Peer Wellness Coaches use one-on-one coaching, self-management workshops, and wellness-related activities to equip individuals with the knowledge, skills, resources, and motivation needed to manage their own physical and mental health symptoms.

Our evidence-informed support is based on the principle that confident, knowledgeable individuals practicing self-management will be empowered to live a life of purpose and well-being.

One-on-One Coaching

One-on-One Coaching consists of weekly sessions with a Peer Wellness Coach for eight weeks. This service provides opportunities for peer support and setting goals within the 8 Dimensions of Wellness: emotional, physical, occupational, social, spiritual, intellectual, environmental, and financial wellness. The program includes:

  • Individual Wellness Assessment in all 8 Dimensions
  • BMI, Weight, and Waist Circumference (optional)
  • Weekly Action Planning

Eligibility

  • Adults with mental illness, substance use disorders, and/or co-occurring disorders
  • Individual is interested in physical health coaching
  • Agrees to complete 8 individual sessions

 

Workshops

Our Peer Wellness Coaches also support people as they work toward their health goals by offering peer workshops that cover a wide range of health-related areas. Our Peer Wellness Coaches offer multiple different programs in a variety of settings, including:

  • Individual outpatient therapy and inpatient programs at behavioral health centers and agencies
  • Mental health crisis units and centers
  • Peer support centers
  • Group homes
  • Other settings for people living with mental health and/or co-occurring disorders

Eligibility

  • For groups of adults with mental illness, substance use disorders, and/or co-occurring disorders
  • Group is interested in physical health coaching
  • Group size limits apply (determined by the workshop topic)
  • Group agrees to complete 6-8 two-hour workshop sessions (1/week); shorter class options available depending on topic

DIMENSIONS: Tobacco Free

A 6-week workshop developed by the University of Colorado Anschutz Medical Campus, School of Medicine. This program helps clients on their tobacco and nicotine cessation journey (including vaping and smokeless tobacco) by providing the latest research, a wide range of information, and support regarding quit methods.

Chronic Pain Self-Management
Program (CPSMP)

A 6-week workshop originally developed at Stanford University, along with the Self-Management Resource Center, The CPSMP is a lay-led participant education program offered in communities in the United States and other countries. The program provides information and teaches practical skills for self-managing chronic pain and assists individuals as they develop the confidence and motivation they need to cope with challenges.

Matter of Balance (MOB)

An 8-week workshop developed at the Roybal Center at Boston University, A Matter of Balance is a program designed to reduce the fear of falling and increase activity levels among older adults. Participants learn to view falls as controllable, set goals for increasing activity, make changes to reduce fall risk at home, and exercise to increase strength and balance.

Mindful Recovery

A 10-session series focused on mindfulness practice and its effects on the brain and nervous system. The program includes guided discussions that reinforce the recovery process.

Diabetes Self-Management
Program (DSMP)

A 6-week workshop originally developed at Stanford University, along with the Self-Management Resource Center, the DSMP a lay-led participant education programs offered in communities in the United States and other countries. The DSMP provides information and teaches practical skills for self-managing diabetes, assisting individuals as they develop the confidence and motivation they need to cope with challenges.

Chronic Disease Self-Management Program (CDSMP)

A 6-week workshop developed at Stanford University in partnership with the Self-Management Resource Center. This lay-led participant education program provides information and teaches practical skills for managing chronic illnesses. It helps individuals build confidence and motivation to manage the challenges of living with a chronic health condition.

Enhancing Immune Health (EIH)

A 5-module workshop that teaches participants how the immune system works and how to apply practical skills for better immune health. Topics include:

  • Reducing vulnerability to colds, flu, and infections through a healthier lifestyle
  • Adequate sleep, less stress, and a well-balanced diet
  • Immunity aids backed by research
  • The role of vaccinations and annual screenings in immune health
  • Goal-setting to improve sleep, reduce stress, and choose foods/supplements that support immune function

DIMENSIONS: Well Body (WB)

A 6-week workshop, The Well Body program teaches information and skills that can be used to build a healthy lifestyle and maintain a healthy weight. This program is designed to help people envision and achieve their personal Well Body goals.

Some groups are particularly at-risk for weight gain and lifestyle-related issues. This may be because of limited access to healthy, affordable food, difficulties in being physically active, or because of other health conditions or medications that contribute to weight gain. In addition, many at-risk populations are under-served by health promotion and weight management services. The Well-Body program provides tailored information designed to meet the needs of priority populations.

First Aid Arts

Based on neurological and psychological research, this program promotes resilience and reduces post-traumatic stress symptoms through arts-based psychosocial resources.

  • Level One: Participants learn the impact of trauma on the brain, body, relationships, spirit, and community. They identify trauma-related behaviors, understand fight-flight-freeze responses, and incorporate arts into self-care routines.
  • Level Two: Participants facilitate intentional, trauma-informed, multi-modal arts-based activities over an 8-week period using the tools learned in Level One.

Nutrition and Exercise for Wellness and Recovery (NEW-R)

An 8-week workshop developed by the University of Illinois at Chicago (UIC) Center on Integrated Health Care and Self-Directed Recovery. This evidence-based program focuses solely on nutrition and physical activity guidance to help people living with mental illnesses and/or co-occurring disorders lose weight.
