- If you are a service provider and are looking to refer someone to Park Center, please fill out the short form below.
- To ensure a smooth intake process, please include any available medical and mental health records along with a Release of Information (ROI) for the member’s Mental Health provider. Completed ROI’s are essential to our intake process.
- Our Intake Coordinator will reach out to you directly, within 24 business hours.
