Park Center is able to receive referrals from any service provider in the community. If you are a service provider and are looking to refer someone to Park Center, please fill out the short form below. Our Intake Coordinator will reach out to you directly, within 24 business hours.

To ensure a smooth intake process, please include any available medical and mental health records along with a Release of Information (ROI) for the member's Mental Health provider. You can use our ROI form, found at the link below. Completed ROI’s are essential to our intake process.
