Park Center is able to receive referrals from any service provider in the community. If you are a service provider and are looking to refer your client or patient to Park Center, please fill out the short form below. Our Admissions Coordinator will reach out to you directly, within 24 business hours.

If you would like to make a referral to our drug and alcohol treatment program, dial 615-242-3576 and press 2.

If you would like to make a referral to any of the following programs: PSR Day Program, Supported Employment, SOAR, or Supported Housing, dial 615-242-3576 and press 3.
